PS 290 IS A ZONED SCHOOL:
We must enroll students who live in our zone for Kindergarten through 5th grade. You cannot register until you live in our school zone.

For 3-K and Pre-K enrollment, zoned families and families within the broader District 2 are invited to apply but zoned families receive priority admissions.

Our zone: please check the zone map above, BUT note that only the interior of the periphery drawn is zoned for PS 290; in other words, our zone includes only the NORTH side of 79th St. and the SOUTH side of 84th St. and so forth.

Your zoned school is determined by your official home address, not your place of employment. If you live outside of the PS 290 zone, you cannot use the address of a family member, friend, or employer who lives in the zone. We verify all addresses.  ​

REQUIRED DOCUMENTATION

IF your child receives an offer for a seat at our school, WE will provide you with registration paperwork to complete. For your planning purposes, to get an idea of the paperwork requirements, please scroll further down.

PLAN AHEAD: we require a Health Form from a recent in-person physical exam by a New York physician (form is below) AND your child's full immunization history from your pediatrician.

If your child has allergies, asthma, or might require medication during school you must submit a completed Medication Administration Form (MAF) from your doctor before school starts.

NYC Public Schools adhere to federal and state laws and regulations protect the privacy of your children's education records and personally identifiable information, or "PII."

Family Educational Rights and Privacy Act (FERPA) provides parents and students certain rights with respect to students’ education records and any personally identifiable information (PII).

Children’s Online Privacy Protection Act (COPPA) imposes requirements of websites and online services regarding collection of information of a child under age 13.

Protection of Pupil Rights Amendment (PPRA) protects student privacy.

Individuals with Disabilities Education Act (IDEA)

REQUIRED DOCUMENTATION

If you are applying your child to PS 290, you must reach out to the school directly. We will provide you with the required documentation for completion. For your planning purposes, to give you an idea of what is required, please see below. There might be additional DOE forms that our school will provide.

In brief, you will need to provide:
(1) Proof of home address x 2, such as lease and recent utility bill;
(2) Proof of the child's age, such as passport or birth certificate;
(3) Completed Health Form (attached below) AND full immunization history from your pediatrician (SEE NY State Requirements). Note: this must be following an in-person exam and from a NY State licensed physician. 
(4) Four Department of Education forms (attached below);
(5) Emergency Contact Form (attached below) - not needed for spring pre-registration but required before the start of school.

Registration documentation details: 

  1. Your child. To begin the pre-registration process, your child must be residing at the address used to apply.

  2. Your offer letter/email from the DOE or our school

  3. The original and a copy of your child’s birth certificate, passport, or record of baptism which includes the date of birth, or other official documents of age (See Chancellor's Regulation for documents accepted for proof of age)

  4. Immunization records for your child

  5. Copy of your child's Individualized Education Program (IEP) and/or 504 Accommodation Plan (if applicable and available)

  6. Originals and copies of any two of the following documents that will serve your proof of address:

    • A lease agreement, deed or mortgage statement for the residence

    • A residential utility bill (gas or electric) in the resident’s name issued by a utility company (e.g., National Grid or Con Edison), must be dated within the past 60 days

    • A bill for cable television services provided to the residence; must include the name of the parent and the address of the residence and be dated within the past 60 days

    • Documentation or letter on letterhead from a federal, state, or local government agency, including the IRS, the City Housing Authority, Human Resources Administration, the Administration for Children’s Services (ACS), or an ACS subcontractor or the federal Office of Refugee Settlement, indicating the resident’s name and address, must be dated within the past 60 days

    • A current property tax bill for the residence

    • A water bill for the residence dated within the past 90 days

    • Rent receipt which includes the address of the residence must be dated within the past 60 days

    • State, city, or other government-issued identification (including an IDNYC card), which has not expired and includes the address of residence

    • Income tax form for the last calendar year

    • Official NYS Driver’s License or learner’s permit which has not expired

    • Official payroll documentation from an employer-issued within the past 60 days such as a pay stub with home address, a form submitted for tax withholding purposes or payroll receipt (a letter on the employer’s letterhead is not adequate); must include home address and be dated within the past 60 days

    • Voter registration documents, which include the name of the parent and the address of residence

    • Unexpired membership documents based upon residency (e.g., neighborhood residents’ association), which include the name of the parent and the address of residence

    • Evidence of custody of the child, including but not limited to judicial custody orders or guardianship papers documents issued within the past 60 days with the name of child and address of residence

Please complete these four forms required by the Department of Education (click on the document attached and download/print)

AND full immunization history from your child's pediatrician. Note, the health form must be completed by a New York State licensed physician AND based on a full physical exam within the past 12 months.
