Required Documents for All Students

REQUIRED DOCUMENTS FOR EACH STUDENT EVERY YEAR

MNS Families:

  ⭑ Make sure you have sent in your Emergency Contact Form (click below)! This is required for every student each year.
  ⭑ Be sure to complete your School Lunch Form each year, regardless if your child has school lunch or brings their own. This impacts school funding. You're not required to provide any personal information. 
  ⭑ Medication: If your child has allergies, asthma, or might require medication during school you must submit a completed Medication Administration Form (MAF) from your doctor before school starts. All forms and translations can be found on the DOE website here:
